RESUMO

RATIONALE AND OBJECTIVES: T2-weighted imaging in at least two orthogonal planes is recommended for assessment of the uterus. To determine whether a convolutional neural network-based algorithm could be used for the re-constructions of uterus axes derived from a 3D SPACE with iterative denoising. MATERIALS AND METHODS: 50 patients aged 18-81 (mean: 42) years who underwent an MRI examination of the uterus participated voluntarily in this prospective study after informed consent. In addition to a standard MRI pelvis protocol, a 3D SPACE research application sequence was acquired in sagittal orientation. Reconstructions for both the cervix and the cavum in the short and long axes were performed by a research trainee (T), an experienced radiologist (E), and the prototype software (P). In the next step, the reconstructions were evaluated anonymously by two experienced readers according to 5-point-Likert-Scales. In addition, the length of the cervical canal, the length of the cavum and the distance between the tube angles were measured on all reconstructions. Interobserver agreement was assessed for all ratings. RESULTS: For all axes, significant differences were found between the scores of the reconstructions by research T, E and P. P received higher scores and was preferred significantly more often with the exception of the comparison of the reconstruction Cervix short of E (Cervix short: P vs. T: p = 0.02; P vs. E: p = 0.26; Cervix long: P vs. T: p = 0.01; P vs. E: p < 0.01; Cavum short: P vs. T: p = 0.01; P vs. E: p = 0.02; Cavum long: P vs. T: p < 0.01; P vs. E: p < 0.01). Regarding the measured diameters, (length of cervical canal/cavum/distance between tube angles) significantly larger diameters were recorded for P compared to E and T (Cervix long (mm): T: 25.43; E: 25.65; P: 26.65; Cavum short (mm): T: 26.24; E: 25.04; P: 27.33; Cavum long (mm): T: 31.98; E: 32.91; P: 34.41; P vs. T: p < 0.01); P vs. E: p = 0.04). Moderate to substantial agreement was found between Reader 1 and Reader 2 (range: 0.39-0.67). CONCLUSION: P was able to reconstruct the axes at least as well as or better than E and T. P could thereby lead to workflow facilitation and enable more efficient reporting of uterine MRI.

RESUMO

We present a deep learning method for the segmentation of new lesions in longitudinal FLAIR MRI sequences acquired at two different time points. In our approach, the 3D volumes are processed slice-wise across the coronal, axial, and sagittal planes and the predictions from the three orientations are merged using an optimized voting strategy. Our method achieved best F1 score (0.541) among all participating methods in the MICCAI 2021 challenge Multiple sclerosis new lesions segmentation (MSSEG-2). Moreover, we show that our method is on par with the challenge's expert neuroradiologists: on an unbiased ground truth, our method achieves results comparable to those of the four experts in terms of detection (F1 score) and segmentation accuracy (Dice score).

OBJECTIVES: To investigate the effect of compressed SENSE (CS), an acceleration technique combining parallel imaging and compressed sensing, on potential bias and precision of brain volumetry and evaluate it in the context of normative brain volumetry. MATERIALS AND METHODS: In total, 171 scans from scan-rescan experiments on three healthy subjects were analyzed. Each subject received 3D-T1-weighted brain MRI scans at increasing degrees of acceleration (CS-factor = 1/4/8/12/16/20/32). Single-scan acquisition times ranged from 00:41 min (CS-factor = 32) to 21:52 min (CS-factor = 1). Brain segmentation and volumetry was performed using two different software tools: md.brain, a proprietary software based on voxel-based morphometry, and FreeSurfer, an open-source software based on surface-based morphometry. Four sub-volumes were analyzed: brain parenchyma (BP), total gray matter, total white matter, and cerebrospinal fluid (CSF). Coefficient of variation (CoV) of the repeated measurements as a measure of intra-subject reliability was calculated. Intraclass correlation coefficient (ICC) with regard to increasing CS-factor was calculated as another measure of reliability. Noise-to-contrast ratio as a measure of image quality was calculated for each dataset to analyze the association between acceleration factor, noise and volumetric brain measurements. RESULTS: For all sub-volumes, there is a systematic bias proportional to the CS-factor which is dependent on the utilized software and subvolume. Measured volumes deviated significantly from the reference standard (CS-factor = 1), e.g. ranging from 1 to 13% for BP. The CS-induced systematic bias is driven by increased image noise. Except for CSF, reliability of brain volumetry remains high, demonstrated by low CoV (< 1% for CS-factor up to 20) and good to excellent ICC for CS-factor up to 12. CONCLUSION: CS-acceleration has a systematic biasing effect on volumetric brain measurements.

Background Functional techniques like diffusion-weighted imaging (DWI) are gaining more and more importance in liver magnetic resonance imaging (MRI). Diffusion kurtosis imaging (DKI) is an advanced technique that might help to overcome current limitations of DWI. Purpose To evaluate DKI for the differentiation of hepatic lesions in comparison to conventional DWI at 3 Tesla. Material and Methods Fifty-six consecutive patients were examined using a routine abdominal MR protocol at 3 Tesla which included DWI with b-values of 50, 400, 800, and 1000 s/mm2. Apparent diffusion coefficient maps were calculated applying a standard mono-exponential fit, while a non-Gaussian kurtosis fit was used to obtain DKI maps. ADC as well as Kurtosis-corrected diffusion ( D) values were quantified by region of interest analysis and compared between lesions. Results Sixty-eight hepatic lesions (hepatocellular carcinoma [HCC] [n = 25]; hepatic adenoma [n = 4], cysts [n = 18]; hepatic hemangioma [HH] [n = 18]; and focal nodular hyperplasia [n = 3]) were identified. Differentiation of malignant and benign lesions was possible based on both DWI ADC as well as DKI D-values ( P values were in the range of 0.04 to < 0.0001). Conclusion In vivo abdominal DKI calculated using standard b-values is feasible and enables quantitative differentiation between malignant and benign liver lesions. Assessment of conventional ADC values leads to similar results when using b-values below 1000 s/mm2 for DKI calculation.

BACKGROUND: In the field of experimental stem cell therapy, intra-arterial (IA) delivery yields the best results concerning, for example, migrated cell number at the targeted site. However, IA application also appears to be associated with increased mortality rates and infarction. Since many rodent studies systemically apply 1 × 106 cells, this could also be a consequence of engrafted cell number. The aim of this study was therefore to investigate the effect of different doses of adipose tissue-derived stem cells (ASCs) on engraftment rates and stroke outcome measured in vivo using 9.4-T high-field magnetic resonance imaging (MRI). METHODS: Male Wistar rats (n = 43) underwent a middle cerebral artery occlusion (MCAo) for 45 or 90 min, followed by IA delivery of either saline or 1 × 106, 3 × 105, or 5 × 104 ASCs pre-labelled with very small superparamagnetic iron oxide particles (VSOPs). MRI (9.4-T) analysis was performed 48 h and 9 days post-MCAo. Lesion volumes were assessed by analysis of T2-weighted images and cell signal tracking showing cell engraftment and active cell migration by an improved T2*-analysis. RESULTS: The ASC-derived signal intensity increased in the affected hemisphere 48 h post MCAo with injected cell number (p < 0.05). The analysis of stroke volumes revealed an increased infarction after injection of 1 × 106 ASCs compared to controls or application of 5 × 104 ASCs (p < 0.05). At 9 days post-MCAo, injection of 3 × 105 ASCs resulted in reduced infarct volumes (p < 0.05). Correspondingly, MRI analysis revealed no changes in cell numbers between both MRI examinations but showed active ASC migration to the site of infarction. CONCLUSION: Our results confirm that IA injection is an efficient way of targeting damaged brain tissue but its usefulness strongly depends on the right dose of delivered stem cells since this factor has a strong influence on migration rate and infarct volume, with better results for doses below 1 × 106 cells. Future challenges will include the determination of therapeutic doses for best cellular engraftment and stroke outcome.

The invasive weed Ambrosia artemisiifolia (common ragweed) constitutes a great threat to public health and agriculture in large areas of the globe. Climate change, characterized by higher temperatures and prolonged vegetation periods, could increase the risk of establishment in northern Europe in the future. However, as the species is a short-day plant that requires long nights to induce bloom formation, it might still fail to produce mature seeds before the onset of winter in areas at northern latitudes characterized by short summer nights. To survey the genetic variation in flowering time and study the effect of latitudinal origin on this trait, a reciprocal common garden experiment, including eleven populations of A. artemisiifolia from Europe and North America, was conducted. The experiment was conducted both outside the range limit of the species, in Sweden and within its invaded range, in Croatia. Our main hypothesis was that the photoperiodic-thermal requirements of A. artemisiifolia constitute a barrier for reproduction at northern latitudes and, thus, halts the northern range shift despite expected climate change. Results revealed the presence of a north-south gradient in flowering time at both garden sites, indicating that certain European populations are pre-adapted to photoperiodic and thermal conditions at latitudes up to, at least, 60° N. This was confirmed by phenological recordings performed in a region close to the northern range limit, the north of Germany. Thus, we conclude that there exists a high risk for establishment and spread of A. artemisiifolia in FennoScandinavia in the near future. The range shift might occur independently of climate change, but would be accelerated by it.

BACKGROUND: Urban green spaces can harbor a considerable species richness of plants and animals. A few studies on single species groups indicate important habitat functions of cemeteries, but this land use type is clearly understudied compared to parks. Such data are important as they (i) illustrate habitat functions of a specific, but ubiquitous urban land-use type and (ii) may serve as a basis for management approaches. NEW INFORMATION: We sampled different groups of plants and animals in the Weißensee Jewish Cemetery in Berlin (WJC) which is one of the largest Jewish cemeteries in Europe. With a total of 608 species of plants and animals, this first multi-taxon survey revealed a considerable biological richness in the WJC. In all, 363 wild-growing vascular plant, 72 lichen and 26 bryophyte taxa were recorded. The sampling also yielded 34 bird and 5 bat species as well as 39 ground beetle, 5 harvestman and 64 spider species. Some species are new records for Berlin.

INTRODUCTION: Stroma reaction leading to fibrosis is the most characteristic histopathological feature of both pancreatic carcinoma and chronic pancreatitis with increased fibrosis compared with healthy pancreatic tissue and further increased fibrosis during radiochemotherapy. Recent studies using intravoxel incoherent motion-derived parameters did not show differences for structural diffusion constant D between these 2 diseases. The aim of this study was to verify the hypothesis that D correlates with the histopathological grade of fibrosis in pancreatic lesions. MATERIALS AND METHODS: We included 15 patients with histopathologically proven pancreatic carcinoma and 9 patients with histopathologically proven focal chronic pancreatitis. Diffusion-weighted magnetic resonance imaging was performed using 10 b values between 25 and 800 s/mm² before surgery. We calculated the apparent diffusion coefficient and the intravoxel incoherent motion-derived parameters D and f within tumors and focal chronic pancreatitis. The resected tissue was evaluated with regard to the grade of fibrosis. RESULTS: Fourteen patients were found to have moderate fibrosis and 10 patients had severe fibrosis. The difference between the D values for the moderate and severe fibrosis was significant with mean (SD) D value of 1.02 × 10⁻³ (0.48 × 10⁻³ mm/s) and mean (SD) D of 1.22 × 10⁻³ (0.76 × 10⁻³) mm²/s. There were no significant differences for the f and ADC values. CONCLUSIONS: Contrary to our hypothesis, D rises from moderate to severe fibrosis. It seems that cellular complexes surrounded by fibrosis provide more structural limitations than does fibrosis alone. Our data suggest that D is not intuitively related to the degree of fibrosis. Compared with healthy tissue, D is reduced in moderate fibrosis but increases when severe fibrosis is present.

Early recognition of and differential diagnosis between pancreatic cancer and chronic pancreatitis is an important step in successful therapy. Parameters of the IVIM (intra-voxel incoherent motion) theory can be used to differentiate between those lesions. The objective of this work is to evaluate the effects of rigid image registration on IVIM derived parameters for differentiation of pancreatic lesions such as pancreatic cancer and solid mass forming pancreatitis. The effects of linear image registration methods on reproducibility and accuracy of IVIM derived parameters were quantified on MR images of ten volunteers. For this purpose, they were evaluated statistically by comparison of registered and unregistered parameter data. Further, the perfusion fraction f was used to differentiate pancreatic lesions on eleven previously diagnosed patient data sets. Its diagnostic power with and without rigid registration was evaluated using receiver operating curves (ROC) analysis. The pancreas was segmented manually on MR data sets of healthy volunteers as well as the patients showing solid pancreatic lesions. Diffusion weighted imaging was performed in 10 blocks of breath-hold phases. Linear registration of the weighted image stack leads to a 3.7% decrease in variability of the IVIM derived parameter f due to an improved anatomical overlap of 5%. Consequently, after registration the area under the curve in the ROC-analysis for the differentiation approach increased by 2.7%. In conclusion, rigid registration improves the differentiation process based on f-values.

BACKGROUND: A new thromboembolic animal model showed beneficial effects of t-PA with an infarct volume reduction of 36.8% in swiss mice. Because knock-out animal experiments for stroke frequently used C57BL76 mice we evaluated t-PA effects in this mouse strain and measured infarct volume and vascular recanalisation in-vivo by using high-field 9.4 T MRI and a 1H surface cryo coil. METHODS: Clot formation was triggered by microinjection of murine thrombin into the right middle cerebral artery (MCA). Animals (n = 28) were treated with 10 mg/kg, 5 mg/kg or no tissue plasminogen activator (t-PA) 40 min after MCA occlusion. For MR-imaging a Bruker 9.4 T animal system with a 1H surface cryo probe was used and a T2-weighted RARE sequence, a diffusion weighted multishot EPI sequence and a 3D flow-compensated gradient echo TOF angiography were performed. RESULTS: The infarct volume in animals treated with t-PA was significantly reduced (0.67 ± 1.38 mm3 for 10 mg/kg and 10.9 ± 8.79 mm3 for 5 mg/kg vs. 19.76 ± 2.72 mm3 ; p < 0.001) compared to untreated mice. An additional group was reperfused with t-PA inside the MRI. Already ten minutes after beginning of t-PA treatment, reperfusion flow was re-established in the right MCA. However, signal intensity was lower than in the contralateral MCA. This reduction in cerebral blood flow was attenuated during the first 60 minutes after reperfusion. 24 h after MCA occlusion and reperfusion, no difference in signal intensity of the contralateral and ipsilateral MCAs was observed. CONCLUSIONS: We confirm a t-Pa effect using this stroke model in the C57BL76 mouse strain and demonstrate a chronological sequence MRI imaging after t-PA using a 1H surface cryo coil in a 9.4 T MRI. This setting will allow testing of new thrombolytic strategies for stroke treatment in-vivo in C57BL76 knock-out mice.

BACKGROUND: To retrospectively and prospectively compare abdominal apparent diffusion coefficient (ADC) values obtained within in a 1.5 T system and 3 T systems with and without dual-source parallel RF excitation techniques. METHODOLOGY/PRINCIPAL FINDINGS: After IRB approval, diffusion-weighted (DW) images of the abdomen were obtained on three different MR systems (1.5 T, a first generation 3 T, and a second generation 3 T which incorporates dual-source parallel RF excitation) on 150 patients retrospectively and 19 volunteers (57 examinations total) prospectively. Seven regions of interest (ROI) were throughout the abdomen were selected to measure the ADC. Statistical analysis included independent two-sided t-tests, Mann-Whitney U tests and correlation analysis. In the DW images of the abdomen, mean ADC values were nearly identical with nonsignificant differences when comparing the 1.5 T and second generation 3 T systems in all seven anatomical regions in the patient population and six of the seven in the volunteer population (p>0.05 in all distributions). The strength of correlation measured in the volunteer population between the two scanners in the kidneys ranged from r = 0.64-0.88 and in the remaining regions (besides the spleen), r>0.85. In the patient population the first generation 3 T scanner had different mean ADC values with significant differences (p<0.05) compared to the other two scanners in each of the seven distributions. In the volunteer population, the kidneys shared similar ADC mean values in comparison to the other two scanners with nonsignificant differences. CONCLUSIONS/SIGNIFICANCE: A second generation 3 T scanner with dual-source parallel RF excitation provides nearly identical ADC values compared with the 1.5 T imaging system in abdominal imaging.

PURPOSE: The objective of this work was to evaluate the diagnostic performance of the intravoxel incoherent motion (IVIM) model to differentiate between healthy and malignant prostate tissue. MATERIALS AND METHODS: Regions of interest were drawn in healthy and cancerous tissue of 13 patients with histologically proven prostate carcinoma and fitted to a monoexponential model [yielding the apparent diffusion coefficient (ADC)] and the IVIM signal equation (yielding the perfusion fraction f, the diffusion constant D and the pseudodiffusion coefficient of perfusion D⁎). Parameter maps were calculated for all parameters. RESULTS: The ADC, D and f were significantly (P<.005) lowered in cancerous tissue (1.01±0.22 µm(2)/ms, 0.84±0.19 µm(2)/ms and 14.27±7.10%, respectively) compared to benign tissue (1.49±0.17 µm(2)/ms, 1.21±0.22 µm(2)/ms and 21.25±8.32%, respectively). Parameter maps of D and f allowed for a delineation of the tumor, but showed higher variations compared to the ADC map. CONCLUSION: Apparent diffusion coefficient maps provide better diagnostic performance than IVIM maps for tumor detection. However, the results suggest that the reduction of the ADC in prostate cancer stems not only from changes in cellularity but also from perfusion effects. IVIM imaging might hold promise for the diagnosis of other prostatic lesions.

The intravoxel incoherent motion (IVIM) theory provides a framework for the separation of perfusion and diffusion effects in diffusion-weighted imaging (DWI). To measure the three free IVIM parameters, DWIs with several diffusion weightings b must be acquired. To date, the used b value distributions are chosen heuristically and vary greatly among researchers. In this work, optimal b value distributions for the three parameter fit are determined using Monte-Carlo simulations for the measurement of a low, medium and high IVIM perfusion regime. The first 16 b values of a b value distribution, which was optimized to be appropriate for all three regimes, are {0, 40, 1000, 240, 10, 750, 90, 390, 170, 10, 620, 210, 100, 0, 530 and 970} in units of seconds per square meter. This distribution performed well for all organs and outperformed a distribution frequently used in the literature. In case of limited acquisition time, the b values should be chosen in the given order, but at least 10 b values should be used for current clinical settings. The overall parameter estimation quality depends strongly and nonlinearly on the signal-to-noise ratio (SNR): it is essential that the SNR is considerably higher than a critical SNR. This critical SNR is about 8 for medium and high IVIM perfusion and 50 for the low IVIM perfusion regime. Initial in vivo IVIM measurements were performed in the abdomen and were in keeping with the numerically simulated results.

Diffusion-based intravoxel incoherent motion imaging has recently gained interest as a method to detect and characterize pancreatic lesions, especially as it could provide a radiation- and contrast agent-free alternative to existing diagnostic methods. However, tumor delineation on intravoxel incoherent motion-derived parameter maps is impeded by poor lesion-to-pancreatic duct contrast in the f-maps and poor lesion-to-vessel contrast in the D-maps. The distribution of the diffusion and perfusion parameters within vessels, ducts, and tumors were extracted from a group of 42 patients with pancreatic adenocarcinoma. Clearly separable combinations of f and D were observed, and receiver operating characteristic analysis was used to determine the optimal cutoff values for an automated segmentation of vessels and ducts to improve lesion detection and delineation on the individual intravoxel incoherent motion-derived maps. Receiver operating characteristic analysis identified f = 0.28 as the cutoff for vessels (Area under the curve (AUC) = 0.901) versus tumor/duct and D = 1.85 µm(2) /ms for separating duct from tumor tissue (AUC = 0.988). These values were incorporated in an automatic segmentation algorithm and then applied to 42 patients. This yielded clearly improved tumor delineation compared to individual intravoxel incoherent motion-derived maps. Furthermore, previous findings that indicated that the f value in pancreatic cancer is strongly reduced compared to healthy pancreatic tissue were reconfirmed.

RATIONALE AND OBJECTIVES: MRI of the prostate offers the possibility to localize and stage prostate cancer and may improve detection of disease. Currently, T2-weighted images and spectroscopy are the most commonly used MRI techniques. To assess the value of prostate MRI and its different modalities in the process of diagnosis, the currently available MRI techniques were compared. MATERIALS AND METHODS: 16 patients were examined on a 1.5 T MR system. All patients underwent the same MR protocol using an endorectal coil: T2-weighted triplanar turbo-spin-echo (TSE), axial echo-planar diffusion-weighted imaging (DWI), 3D chemical-shift imaging MR spectroscopy (MRS) and axial dynamic-contrast-enhanced TurboFLASH (DCE). Parametric maps of the choline+creatine/citrate ratio (CC-CR), apparent diffusion coefficient (ADC) and plasma flow/mean transit time (PF/MTT) were calculated. Additionally, average time for reading and scanning were evaluated. As reference, biopsy results were used. RESULTS: Sensitivity/specificity were 50.0-85.7%/44.4-72.2% for the T2 weighted images, 78.6-100.0%/38.9-55.6% for the ADC maps, 71.4-85.7%/44.4-55.6% for the PF/MTT maps and 64.3-78.6%/50.0-77.8% for the CC-CR. Average scan and reading time were 8:46/1:54 min for T2, 1:28/3:17 min for DWI, 8:41/2:12 min for DCE and 11:36/3:47 for spectroscopy. CONCLUSION: We found no significant differences in accuracy between the modalities. We observed DWI to be advantageous in examination and reading compared to DCE and MRS, therefore it might be the preferred modality when a shortened protocol is needed.

The Roseobacter group and SAR11 clade constitute high proportions of the marine bacterioplankton, but only scarce information exists on the abundance of distinct populations of either lineage. Therefore, we quantified the abundance of the largest cluster of the Roseobacter group, the RCA (Roseobacter clade affiliated) cluster together with the SAR11 clade by quantitative PCR in the southern and eastern North Sea. The RCA cluster constituted up to 15 and 21% of total bacterial 16S ribosomal RNA (rRNA) genes in September 2005 and May 2006, respectively. At a few stations, the RCA cluster exceeded the SAR11 clade, whereas at most stations, SAR11 constituted higher fractions with maxima of 37%. In most samples, only one RCA ribotype was detected. RCA abundance was positively correlated with phaeopigments, chlorophyll, dissolved and particulate organic carbon (POC), turnover rates of dissolved free amino acids (DFAAs), temperature, and negatively correlated with salinity. The SAR11 clade was only correlated with POC (negatively, May) and with DFAA turnover rates (positively, September). An abundant RCA strain, 'Candidatus Planktomarina temperata', was isolated from the southern North Sea. This strain has an identical 16S rRNA gene sequence to the dominant RCA ribotype. Detection of the pufM gene, coding for a subunit of the reaction center of bacteriochlorophyll a, indicates the potential of the isolate for aerobic anoxygenic photosynthesis. Our study shows that a distinct population of the RCA cluster constitutes an abundant bacterioplankton group in a neritic sea of the temperate zone and indicates that this population has an important role during decaying phytoplankton blooms.

PURPOSE: To determine which of the quantitative parameters obtained from intravoxel incoherent motion diffusion weighted imaging (DWI) is the most significant for the differentiation between pancreatic carcinoma and mass-forming chronic pancreatitis. MATERIALS AND METHODS: Twenty-nine patients with pancreatic masses were included, 9 proved to have a mass-forming pancreatitis and 20 had a pancreatic carcinoma. The patients were studied using intravoxel incoherent motion DWI with 11 b-values and the apparent diffusion coefficient (ADC), the true diffusion constant (D) and the perfusion fraction (f) were calculated. The diagnostic strength of the parameters was evaluated using receiver operating characteristic analysis. RESULTS: The ADC in chronic pancreatitis was higher than in pancreatic carcinoma with significant differences at b = 50, 75, 100, 150, 200, 300 s/mm (ADC50 = 3.17 ± 0.67 vs. 2.55 ± 1.09, ADC75 = 2.46 ± 0.4 vs. 1.93 ± 0.52, ADC100 = 2.28 ± 0.48 vs. 1.73 ± 0.45, ADC150 = 1.97 ± 0.26 vs. 1.63 ± 0.40, ADC200 = 1.98 ± 0.24 vs. 1.53 ± 0.28, and ADC300 = 1.76 ± 0.19 vs. 1.46 ± 0.31 × 10(-3) mm2/s). No significant differences were found at b = 25, 400, 600, and 800 s/mm (ADC25 = 4.69 ± 0.65 vs. 4.04 ± 1.35, ADC400 = 1.57 ± 0.21 vs. 1.37 ± 0.30, ADC600 = 1.38 ± 0.18 vs. 1.24 ± 0.25, and ADC800 = 1.27 ± 0.10 vs. 1.18 ± 0.19 × 10(-3) mm2/s) nor using ADCtot (1.42 ± 0.23 vs. 1.28 ± 0.12 × 10(-3) mm2/s). The perfusion fraction f was significantly higher in pancreatitis compared with pancreatic carcinoma (16.3% ± 5.30% vs. 8.2% ± 4.00%, P = 0.0001). There was no significant difference between groups for D (1.07 ± 0.224 × 10(-3) mm2/s for chronic pancreatitis and 1.09 ± 0.3 × 10(-3) mm2/s for pancreatic carcinoma, P = 0.66). For f, the highest area under the curve (0.894) and combined sensitivity (80%) and specificity (89.9%) were found. CONCLUSIONS: There were significant differences in ADC50-300 between chronic pancreatitis and pancreatic carcinoma. Because D is not significantly different between groups, differences in ADC can be attributed mainly to differences in perfusion. The perfusion fraction f proved to be the superior DWI-derived parameter for differentiation of mass-forming pancreatitis and pancreatic carcinoma.

To investigate the vascular contribution to the measured apparent diffusion coefficient and to validate the Intra Voxel Incoherent Motion theory, the signal as a function of the b-value was measured in the healthy pancreas with and without suppression of the vascular component and under varying echo times (TE = 50, 70, and 100 msec). The perfusion fraction f and the diffusion coefficient D were extracted from the measured DW-data using the original Intra Voxel Incoherent Motion-equation and a modified version of this equation incorporating relaxation effects. First, the perfusion fraction f in the blood suppressed pancreatic tissue decreased significantly (P = 0.03), whereas the diffusion coefficient D did not change with suppression (P = 0.43). Second, the perfusion fraction f increased significantly with increasing echo time (P = 0.0025), whereas the relaxation time compensated perfusion fraction f' showed no significant dependence on TE (P = 0.31). These results verify a vascular contribution to the diffusion weighted imaging measurement at low b values and support the Intra Voxel Incoherent Motion-theory.

Amphotericin B was formulated as nanosuspensions to develop a nanoparticulate brain delivery system. Nanosuspensions were produced with different surfactant solutions by high-pressure homogenization and then characterized by laser diffractometry and photon correlation spectroscopy. Before in vitro and in vivo testing all nanosuspensions were investigated for protein adsorption by two-dimensional polyacrylamide gel electrophoresis to predict brain-targeting capacities. Selected nanosuspensions were tested for amebicidal activity against Balamuthia mandrillaris, an agent of lethal encephalitis. Our results indicate that nanosuspensions coated with polysorbate 80 and sodium cholate markedly increased drug brain delivery and inhibited the parasite in vitro, though less in vivo. OBJECTIVES: To evaluate in detail the diagnostic performance of diffusion-weighted imaging (DWI) to differentiate pancreas carcinoma from healthy pancreas using the apparent diffusion coefficient (ADC) and parameters derived from the intravoxel incoherent motion (IVIM) theory. MATERIALS AND METHODS: Twenty-three patients with pancreas carcinoma and 14 volunteers with healthy pancreas were examined at 1.5 Tesla using a single-shot echo-planar imaging DWI pulse sequence. Eleven b-values ranging from 0 to 800 s/mm2 were used. The acquisition was separated into blocks (b0, b25), (b0, b50),...(b0, b800) and each block was acquired in a single expirational breath-hold (TA = 26 seconds) to avoid motion artifacts. The ADC was calculated for all b-values using linear regression yielding ADC(tot). By applying the IVIM model, which allows for the estimation of perfusion effects in DWI, the perfusion fraction f and the perfusion free diffusion parameter D were calculated. The diagnostic performance of ADC, f and D as a measure for the differentiation between healthy pancreas and pancreatic carcinoma was evaluated with receiver operating characteristics analysis. RESULTS: In the healthy control group, the ADC(tot) ranged from 1.53 to 2.01 microm2/ms with a mean value of 1.71 +/- 0.19 microm2/ms, the perfusion fraction f ranged from 18.5% to 40.4% with a mean value of 25.0 +/- 6.2%, and the diffusion coefficient D from 0.94 to 1.28 microm2/ms with a mean value of 1.13 +/- 0.15 microm2/ms. In patients with pancreas carcinoma, the ADC(tot) ranged from 0.98 to 1.81 microm2/ms with a mean value of 1.31 +/- 0.24 microm2/ms, the perfusion fraction f ranged from 0% to 20.4% with a mean value of 8.59 +/- 4.6% and the diffusion coefficient D from 0.74 to 1.60 microm2/ms with a mean value of 1.15 +/- 0.22 microm2/ms. In comparison to healthy pancreatic tissue, a significant reduction of the perfusion fraction f and of ADC(tot) was found in pancreatic carcinoma (P < 0.00001, 0.0002, respectively). The f value showed more than a 10-fold higher significance level in distinguishing cancerous from normal tissue when compared with the ADC(tot) value. No significant difference in the diffusion coefficient D was observed between the 2 groups (P > 0.5). In the receiver operating characteristic-analyses, the area under curve for f was 0.991 and significantly larger than ADC(tot) (P < 0.05). f had the highest sensitivity, specificity, negative predictive value, and positive predictive value with 95.7%, 100%, 93.3%, and 100%, respectively. CONCLUSIONS: Using the IVIM-approach, the f value proved to be the best parameter for the differentiation between healthy pancreas and pancreatic cancer. The acquisition of several b-values strongly improved the stability of the parameter estimation thus increasing the sensitivity and specificity to 95.7% and 100% respectively. The proposed method may hold great promise for the non invasive, noncontrast-enhanced imaging of pancreas lesions and may eventually become a screening tool for pancreatic cancer.
